Information you submit
A consultation request asks for your name, country or region, preferred contact method, contact details, treatment interest and optional visit dates. Do not include medical records, sensitive health details or photos in this form.
Purpose and access
Your details are used to respond to your request and discuss availability and next steps. Inquiry records are stored in the clinic project database. Public website visitors cannot browse submitted inquiries. A request does not confirm an appointment.

Network information and visit statistics
Where available, the IP address of a consultation request is stored for operational security and request management and is visible only to administrators.
Visit statistics use an IP-based country estimate and a pseudonymous browser-session identifier. Raw IP addresses, names, contact details and detailed treatment-page URLs are not stored in visit statistics, and IPs are not sent to an external lookup service. VPNs, shared networks and proxies may affect country estimates. Previously uncollected IP addresses or visits are not reconstructed or fabricated.
